To request a hazardous material be processed for shipment, please complete and submit the form below 48 hours prior proposed shipping date.  Accurate and complete information provided at the time of request will expedite the shipment.

• In the top right corner, click "New Request/Report"
• A box pops up where you can click on "General Request".
• When you can select a type, please choose Hazardous Materials Shipping Request and fill out the form that appears to the best of your ability.

Please note for Dry Ice shipments, an accurate weight of dry ice that will be shipped with the material must be provided or the package may be rejected by the carrier.  Contact EHS with any questions completing the form.
